NVidia Web Driver crashes on Sierra

I installed Sierra 10.12.5 on my Hack specs as follows:

  • ASUS H170-Plus D3 Motherboard
  • Intel Core i3-6100
  • EVGA GTX 960 4GB
  • 8GB RAM

Everything works, aside from graphics - I can only boot with nv_disable=1. I installed the correct Web driver and used the new NVidia Web flag to enable the web driver, but it boots 3/4 of the way before my monitor says it is going to sleep and then the computer promptly restarts. What's going wrong?
