Sonoma - Optiplex 7050 Micro - Dual Display issues

Hi all,
First time Hackintosher, long time lurker.

I picked up a Dell Optiplex 7050 Micro a few weeks back to do my first Hackintosh project before Apple removes our ability to do so in a year or two.
Everything is working swimmingly on this machine. I first installed Monterey, then upgraded to Sonoma from there because it seemed like the path of least resistance.
Now that Sonoma is running well, the last item I want to get working - before I turn off verbose boot and just use it as a daily driver - is to get dual displays working.

The specs on this machine are:
i5 7500T
16GB DDR4 RAM
HD630 - 1x HDMI, 1x DP out
Optional: DP Micro expansion card (not currently installed)

I've had a bit of a hard time with this, because when following the framebuffer patching guide I was unable to hot plug each display. My HDMI output would work, but as soon as I plug in the DP to check which port number it was, the HDMI display would shut off and the DP display wouldn't get signal.
Eventually (don't ask me how) I managed to get it to boot on the DP display so I could get that port number (HDMI Port 1 Bus 0x05, DP Port 3 Bus 0x06). Once I did that I was able to generate the patch which is currently in my config.plist (below).
That said though, I'm still unable to get dual displays. With the current patch, if I plug both displays in and boot, the bootloader will run through on the DP monitor before it shuts off just prior to the login screen, and the HDMI display doesn't take over - it remains off. Doing so with just the HDMI display plugged in, it boots fine. If I then plug in the DP display, I get the same behaviour where both shut off.

I'm now at a loss for where to go from here, so I'd really appreciate some guidance from the smarter people in the room. My config.plist is here.
