Help with kernel panics

Hey guys. I have a 4770k with Z87X-UD4H and Rx 560. I was originally using Sierra for years and everything was perfect, and then went to shit as soon as I installed Mojave. I have now completely fresh installed Mojave, using a DSDT made for my setup over at Insanely Mac, and used the vanilla guide here for Haswell systems. My kexts include AMDRadeonPro, AppleALC, EFICheckDisabler, FakeSMC, IntelMausiEthernet, Lilu, NoVPAJpeg, USBInjectAll, and WhateverGreen.

Everything is working well with one exception: Occasionally when attempting to wake up from sleep, it does not respond... the computer light turns on, but the display receives no signal. When I hard reboot, I get a message that my system is rebooting from a Kernel Panic. Once it loads into Mojave, this is the diagnostic info it displays:
